This is the puppy purchase agreement. You will receive a signed puppy health guarantee at pickup. Licking county sales tax 7.25% is charged on the purchase price of the puppy unless you are having it shipped out of Ohio. The balance on your puppy is due at pick up time, there is a 4% fee for charge cards and also for PayPal. You can do charge cards on the PayPal site without having a PayPal account, or you can just use your card here on the machine at your pick up date. We do NOT accept personal checks on pickup day. We do accept cash, charge cards or bank cashiers check for the balance due.
You should have already made a deposit for the puppy by the time you fill out the form, but if not, you can use Paypal to do so right here the 4% has already been added to the deposit amount.
